I do not have desktop computer to test your form, but I checked your embedded form on my laptop and your form is visible. However there should be no difference between laptop and desktop computer, when forms are viewed. Do you maybe have different OS on your laptop and desktop computer or you are using different browsers?
Also when I inspected your webpage I saw that you have embedded your form with script code. Maybe there are script conflicts between your webpage scripts and JotForm script. We recommend using of iFrame code. Please try to replace script code with iFrame code to see if that will resolve your issue.
