What happened to my form?
I had a beautiful form that was working perfectly, but now all of the fields seem to have disappeared from the form. I don't know what happened to it or why it changed.

We are sorry for the inconveniences caused.
It looks like that form was somehow updated about two hours ago. We have now reverted the changes with a Revision History Tool. Please re-check the form now.
