I have a two part recommendation form which I would like the applicant to be able to fill out part A and then have another person complete.
Also, I added the page separation widget but it doesn't seem to be working.

I think that this could be achieved by using the Save and Continue Later feature and an autoresponder.
Basically, you will have to add the feature to your form, you will see how to do it on this guide: http://www.jotform.com/help/97-How-to-Save-Forms-and-Continue-Later.
Then, once the first user has finished to fill his/her part of the form, you can add a check box or ther field, your user will have to check the option or fill it to send the autoresponder to your second person, this guide will help you to add an autoresponder: http://www.jotform.com/help/26-Creating-a-Form-Autoresponder.
And this guide will help you to send the email based on the user selection: http://www.jotform.com/help/167-How-to-send-to-a-specific-email-address-based-on-a-selection.

The link that is created to associate the user who completed the first part of the form is, for example…

When that link is forwarded to the person who has to complete the rest of the form, I need the first part to be prepopulated so I created the field map as follows…

I understand that this is to be added to the end of the form url with a “?” in between. Does this go before or after ?session={email} and can the both be used together?

Yes, can both be used, just add all the prepupolated fields after the ?session={email}, but add them with & instead of ?, this should be your URL:

Replace the variable with program, so instead of use https://form.jotform.com/60334761178154?session={email}&Program={Program}...
Use: https://form.jotform.com/60334761178154?session={email}&program={program}...

I tried that and got the same result. I also made sure that the field on both forms are identical.

I want users to have the actual url and not just “click here.” Any further suggestions?

I think that it's not populating on your second form because the options on the drop down "Program" are different, I have copied and pasted the options from your first form to the second one and it's working on my end.
Just make that change, the fields should be prepopulated properly now, do not change anything in your URL, I used the same that you have in your autoresponder and it worked.
