I have a big issue, all the fields in my form have dissapear and apparentlty I've lost all the data

    Good morning,

     

    I have the form https://form.jotform.co/42886889201871 which has had work properly for about a year and a half.

    Today I noticed that all the fields in the form have dissapeared and the data is gone too. The only person that has access to edit or change the layout is me so I'm really worried.

    Please help!

    According to the revision history,  the changes were performed 4 hours ago. The reason this might have happened is that you had some password fields on it. You can revert to a previous version this form, but you must remove the password fields, otherwise, our system will delete them again.
