I was hoping to modify the contents of the PDF file that is available with the notification email.
Can fields be added? Or reordered? What is the basis of the field being included in the PDF?

You can modify the submission PDF by heading to the submissions page and clicking the Get PDF button. Then click on the Customize PDF button.
A new tab will open where you can edit the PDF.
Simply save the report and you're all set.
