Apologies for the inconvenience. I checked your website and it seems related to the number of plugins and scripts that conflicts with the form's own embed code.

I would strongly advise using the iFrame embed code, here's how to get it: http://www.jotform.com/help/148-Getting-the-Form-iFrame-Code

I checked your form directly using the form's URL, which is this one: https://form.jotformeu.com/50205919113346. So far the sections associated with the selected item on "How many Players would you like to register" are showing properly, so I presume the problem is more on the form embedded on your website.

If you are using Wordpress to organize or setup your website, perhaps following this guide would also help: http://www.jotform.com/help/324-A-Guide-To-Forms-For-WordPress-JotForm-Users

I hope that resolves the first three issues.

For the 4th one, I have opened a separate thread as I see that it seems to be unrelated to how you have embedded your form on Wordpress. Please refer to this link instead: http://www.jotform.com/answers/763450. We will address it accordingly. Thank you for your understanding.

I checked your form and I see both fields, allow me some minutes, I will investigate the issue and will get back to you as soon as possible.

    Thank you.

Upon checking your form I found a weird behavior, the fields are under the sections Player Information 2, player Information 3 and Player Information 4, now, you are manipulating these sections to show them based on the selection on the field "How many Players would you like to register?".

This is how the form looks when seeing it in the Form Builder:

This is how the same section looks on the Designer

Now, this is due to a conflict in your conditions, you have deleted some fields, so the conditions are not waiting for those deleted fields to proceed with the actions, I would suggest you to correct all those conditions and then test your form again, based on the logic that I see, they are to show the sections for additional Players.

After doing the changes, your form should work properly, and the fields that you are currently seeing will be shown properly based on the selection.

Do let us know if it works, if not, we will be glad to take other look to your form.
