I see that there was a thread last year, ending on August 5th about the font change that occurs when converting a form to pdf.

I just converted a form to pdf for the first time, and the font and size I chose do not display.  This is very important to my organization because I work with Elders who cannot read the small Times New Roman font that displays once converted to pdf.  Also, our Elders do not use computers, so having a paper form for them to complete is essential.

I look forward to your reply,

    Thank you

Is this the thread you mentioned http://www.jotform.com/answers/535614? Issue on that thread is resolved.

Please note that Fillable PDF form will not look exactly the same as online form. I have created Fillable PDF form from form you provided and it looks like this.

Font color of PDF  form is always black. More about  Fillable PDF form you can find inside of this guide: http://www.jotform.com/help/312-Fillable-PDF-Everything-you-need-to-know-about-JotForm-Fillable-PDF-Creator

You need to use Adobe Acrobat Reader or Foxit to be able to fill and submit Fillable PDF form.

If you want to have paper form then I would suggest you to add print button to your form and print blank form. Inside of this guide you can find out how to add a print button in a form:

Hope this will help. Let us know if you need further assistance.
