Form Failed

    Hello,

    I've provided my employees with this link to our form:  https://form.jotform.com/50275085118958

    and a few people, including myself have tried the link and we keep getting a form failed error.

    Can you help please?

    I'm able to see your form on my end, are you still getting error message?
