I checked your webpage where the form is embedded and I was able to replicate the problem.

I can see that you are using Oembed plugin. It is possible that there are script conflicts between the form and the webpage. I tried cloning the form and did a test submission, it is successful.

Can you please try to re-embed the form using the Iframe method. Please refer to this guide: Getting-the-Form-iFrame-Code

    <iframe id="JotFormIFrame" onload="window.parent.scrollTo(0,0)" allowtransparency="true" src="https://form.jotform.com/12345678912345" frameborder="0" style="width:100%; height:539px; border:none;" scrolling="no"> </iframe>
    <script type="text/javascript"> ......... </script>

If that didn't work, please try the Iframe method without the script tag below it.

    <iframe id="JotFormIFrame" onload="window.parent.scrollTo(0,0)" allowtransparency="true" src="https://form.jotform.com/12345678912345" frameborder="0" style="width:100%; height:539px; border:none;" scrolling="no"> </iframe>

Let us know if it is still not working. Thank you.
