I was able to send a password recovery to the email on file, but I don't have access to the email any longer. Is it possible to have you send me a temporary password?Page URL:

If you do not have email address information, then it would be better to create new account with your email address. You can sign up on this link.
You can then clone form from website you provided to your account and re-embed your cloned form to website. http://www.jotform.com/help/67-Which-Form-Embed-Code-Should-I-Use
