I presume you can embed the script codes on Google Apps. However, JotForm itself does not hold a HIPAA compliance certificate, but you can use JotForm in a HIPAA compliant way. You can check my manager's post about this on this link: http://www.jotform.com/answers/333046-is-JotForm-HIPAA-Compliant.
You can get the embed codes under the "Publish" wizard. I presume this is similar to how you can add an embed code on a web page, here's a guide that you can follow: http://www.jotform.com/help/34-Adding-a-Form-to-a-Web-Page
