In iPad the alignments are different

Hi, I used the form

In iPAD the alignment is not good. In the 2nd row, 2nd column is moved to 3rd column.

I checked it on a screen resolution similar to iPad and it works properly, however, try following this guide: http://www.jotform.com/help/311-How-to-make-Forms-Mobile-Responsive. 

It should display properly your form on mobile devices.
