I'm able now to sign in but as I mentioned before, I had received a notification from you that
I received a form. I looked in my account there is no form anywhere! The Form was for Dana 235.
I would appreciate your help!

I checked your account and in fact, there is no forms, because based on your account history, you have not created any form yet:
Is somebody supposed to share forms with you? If yes, you can share him this guide in order to do it: http://www.jotform.com/help/232-How-to-Share-Forms-with-a-Sub-Account-User, the person basically needs to send you the invitation to your account email address: email@example.com
