I want to add columns so my form is not one question above the next
So I can have the name, address, and phone all on one section side by side. How do I do this?

To achieve that you will need shrink your form fields. Inside of this guide you can more about form field positioning and how to position fields inside of your form:
If you are still unable to position some fields inside of your form, please provide us ID of your form and tell us which fields you want to position in same line.
