I was not able to find your form on webpage you provided, but I assume that this is the form you are working on http://www.jotform.us/form/60378296855167.
If I understood you correctly you are not receiving all submitted data inside of your email Notification.
I have inspected email Notification settings of that form and form field values are not correct.
You will need to set correct field value for each field. Or you can simply create new Email Notification and delete old one. When you create new Notification all fields and values of fields will be automatically added inside of your Email Notification.
If I did not understood you question correctly, please explain in more details what you are trying to achieve and we will be happy to assist you.

That's good to know, thanks, but my original question was in regard to the exam results viewable at http://quizform.jotform.io/. The questions are all in a random order. My email notifications appear correctly.

Please disregard my previous answer, because I did not understood your question correctly. Your email Notification is set like that because you are using quizform app.
I have cloned your form and I was able to replicate mentioned issue when I created form quiz, submitted my form and view results on quizform app.
I will escalate this issue to our developers and we will inform you via this thread once this issue is resolved.

I have checked and it seems that original issue is fixed.
