I checked your email address email@example.com and it is associated with the username AMCox. If you are unable to log in to your account, you can do a password reset.
1. In www.jotform.com, click the "LOG IN" button and choose "Forgot Password".
2. Here you can add the email address associated with the registered account.
3. You should then receive an email with instructions on how to reset the password.
