How can I have the form come over exactly how it looks?

    I need the entire form (without photos) to email with all the wording on the form along with the esignature

    Hello dsetaro,

    If I understand your requirement correctly, you want to show "Text" block you have added in your form in the email alert. Is that correct?

    Yes it is possible to achieve your requirement. You will have to manually add the texts in the notification email alert template. Please check the screenshot below:
