Clients continuously receive an error message stating that the form is unavailable. I have enables the form through the my forms page by selecting enable from the more and the edit form dropbox.. Please help promptly.Page URL:

It's because you have established a limit of submissions, your current limit is 5 and you have received 5 submissions, so the form has reached the limit and this is why it's not allowing more submissions.
Your form settings should look like this if you want to receive unlimited submissions.
