All the fields that are found in the form will appear in the email notification/autoresponder.
Here's a screenshot of a sample form.
Notice that the email notification also has the fields Full Name, E-mail, and Phone Number in them.
When a notification is set up, all the fields found in the form will be displayed in the email. A user can also modify these fields to choose which fields to display. For example, fields that are hidden will not be seen in the email. A user can also remove the fields that they do not want to show in the email.
