I just emailed you about a form with Page Break Issues. The same form also does not submit. The link is attached. None of the fields are required right now as we work through it so it's easier to test.

I checked the page you have linked and made a test submission. I was able to replicate the issue. The submit button was stuck to Please Wait and the form is not submitting.
However, when I opened the form using this link (http://www.jotformpro.com/form/60414450725954), I was able to submit the form without any issue.
To fix the issue with submit, I suggest you use the full iFrame embed code to embed the form to your website. Here's a guide on how to get the iFrame embed code: Getting the Form iFrame Code
Do let us know if the issue persists.

That did indeed work. Just not sure why. We've used jotform for years and we run into this issue from time to time. And it always leads to embedding the form differently. And finding the work around, like this, is time consuming. And while testing the form, we have to constantly update the code, rather than refreshing a page to see the new form edits.
Can you tell me why this is so when all our other forms work with just the embed link?

Sorry for the inconvenience. We are not really sure how you embedded the other forms, but we recommend always using the iFrame code. In some cases using Javascript or the source code, will have conflicts with the form's code and your site. Using iFrame will eliminate this conflicts.
If your other sites are not using iFrame to embed your form, I would modify it so this does not occur.
