My form is not working, people re submitting but the information is not going where it should.

Our form is connected to google to spits out the answers in Google Sheets & Google Folders, however its not working. You are able to complete the form just fine, but the answers are not appearing in google. This happened yesterday and I disconnected the form to google and reconnected and it was fine.... however its not working again today. Is there a reason why? 

You have not to modify the spreadsheet where you are sending your submissions, it could break it.

I cloned your form and integrated it with Google Spreadsheet, I can see it's working as expected, I would suggest you to test it again and as advice above, do not modify the spreadsheet, it could break the integration.
