Can you please clarify the process you were trying to do?
Were you trying to upload a form to your My Forms? What form is it?
There is really no option to upload external forms to your My Forms in your JotForm account.
We will wait for your response.

I inspected your forms and you do not have any upload field on them. Are you trying to import a PDF form using the pdf import tool?
If so are you receiving any error message? Have you tried more than one browser with the same results?
We'll wait for more details to continue.

cheryl256Replied on February 16, 2016 at 4:46 PMYes, I would like to add a form for the applicants to read before
submitting their registration. Is this possible?

You can use the PDF Embedder Widget to add a PDF document to your form that the end user will be able to read/review within the form itself.
link: http://widgets.jotform.com/widget/pdf_embedder
Here is a demo form that uses the widget.
http://form.jotformpro.com/form/51863020628957?
As you can see, there is a PDF embedded on the form that can viewed.
Hope this help. Let us know how we can be of further assistance.
