Calculations Wizard stopped working

This form has been working flawlessly up until Feb 10th, then on a Feb 12th submission, it stopped calculating a summation of numerical entries.  I am not able to figure out why or how to fix it.  I've cloned the form for your testing, as the culprit form is in 'live' use.

For your test, go to "Checklist" (page 2) and type in some numerical entries for the observations questions under PPE (positive and questionable)  It should calculate below.  The calculations should also tally all questions and display a total at the bottom of Tools and Equipment.  These total tally fields are used for an overall % score calculation which is brought back to page 1 under Feedback Session.

Allow me some time to investigate further what could be causing the issue. I'll get back to you shortly.

The calculation of Matrix row values can only work when using the Form Calculation widget.

in the Form Calculation configuration wizard, you can select the rows you wanted to be calculated by inserting them.

You can try modify this on your form and use the Form Calculation widget instead.
