Is there any way to get the entire form to submit?

Asked by kgpost2 on February 15, 2016 at 11:40 AM

I would like to have the entire contract, not just the checkmarks, Q&As, signature.

I tried to get that by putting the entire passage into the checkmark question, which worked.. in that I get the entire passage, but on the submission it's in one very narrow column.

Also, the checkmark itself did not print.. only the words in back of the box: "Check this box to indicate that you understand....."

The person submitting can print the form with their answers & signature, and therefore create a pdf, can I get one too?

    ANY IDEAS??

    Thank you!

When you submit the form, the inputted values are the ones being saved in the submission record. In the check boxes, the value on the check box is the one being recorded and not a check mark.

If you wish to print the form with the layout and just like in the live preview but with the form fields populated, you can send yourself an email notification with the edit link. Here are the steps you can follow:

    1. In your email notification that will be sent to you, add the {edit_link} tag in the email template.

    2. When you receive the notification, click the "Edit Link" button to open it on a new window.

    3. Here you'll be able to view the form just like in the live preview but with the form already prepopulated. You can click the "Print" button from here.

    4. You can save it as a PDF file or print it directly.

We have a PDF submission but I presume you already aware of it and you are looking for the actual layout of the form, just like how it is displayed on the live preview.

Do let us know if I misunderstood your concern.

    One more question....

    2 more things:

    1) How does a person with a mouse sign the form?

    2) After embedding form on my website, all of a sudden it won't submit...just says: Please wait...

It worked before just fine. Hmmmm....  https://www.phawellness.com/patient-services-consent-form/

    thanks again!

It seems like you were editing your posts, I'm not quite sure with the current question or concern you have right now.

For the person uses a mouse, he/she can sign the signature widgets by using the mouse itself. Although it might be difficult for them, unfortunately this is more related to the user's current devices that he/she is using. If he/she has a tablet or smartphone with a stylus, that would also work for the widget, I believe.
