regarding my DSS form with close to 500 submissions. It has been sending correct emails to dss@datasystemsservices.com up until end of 07/2014. It no longer sends the data to us. When I use the test feature of jotform it sends the template but when I use it in my website it does not.
Any help appreciated,

I checked our mail logs and it appears that the emails are successfully sent when using noreply@jotform.com, however there were also failed notifications with the sender e-mail defined as (default).
To fix the issue, I suggest you try using noreply@formresponse.com as the sender email.
Or you can also delete your previous notification and create a new one by following this guide: Setting Up Email Notifications
