Can I create a PDF form that contains data from a customer submitted form?

Customer goes to my form site and fills out request for service form and submits.  The Submit actually sends form1 data to a form2, used in the office. That form2 contains data from form1 and contains additional fields for sales people to fill out....  and further... form2 is a contract proposal and contains a spot for customer signature and date agreeing to proposed work?

Think about a tree company.  Customer goes to website to request a services bid. Request comes into the office email as a service/bid worksheet with some of the blanks already filled in.  The office forwards the bid worksheet to nearest field rep who visits potential customer, opens form on tablet, and completes bid.  Then hands tablet to customer to sign & date.

Can one form display multiple submit buttons with different destinations?  exmple: Form2 above, submit1 to scheduler, submit2 to NOT YET file.

Can I upload an existing PDF form as a background and create form/add fields on top of it where the fields will stay fixed?

    Thank you. - Mark

You can use Condition Logic to use rules that will allow dynamic Submissions to different destinations.

    User guide: Change-Thank-You-URL-upon-a-condition-is-met-

    Example scenario:

Selected option 1 = upon submission, Submits to Page 1 URL

Selected option 2 = upon submission, Submits to Page 2 URL

You can use your own image file as the background of your form using Form Designer.

    Is your PDF an image?

PDFs are usually are documents... so, you may want to get first the image instead from the PDF so you can use it on your form.

Here is a video tutorial how to add background image to your form
