sometimes they work and sometimes not

On first URL you provided, I am getting Page not found (error 404).
On other two URLs your forms are shown. I have also tested both forms and I was able to submit them with out any issues. When I submitted your forms I got redirected to thank you page.
When I inspected webpages you provided I saw that you have embedded your forms with script code. Maybe there are some script conflicts between your website scripts and JotFrom scripts. We recommend using of iFrame code for embedding form to website. Please try to replace script code with iFrame code to see if that will resolve your issue. If this does not help, please provide us correct URL of your first webpage.
Can you also tell us if you are unable to submit your forms or they are now shown on your website or is there something else that is not working correctly?
