I am able to logon to the Windows ap from my Windows 10 machine using Chrome but my IOS 6sPlus keeps coming up with incorrect logon info. Yes, I have double checked the password several times.

Your password should be the same across all platforms. Are you trying to log in to the JotForm site via your iPhone or to the iPhone app?
