Form not displaying properly on IE

http://shop.wagnercompanies.com/quote-request  the following form on our site is not displaying properly on IE and it is not supply our customer's with any sort of error and the form are not able to be submitted.  This is an urgent issue that we need to get resolved.  

I checked your website and I see the same issue. However, I loaded the direct link of the form and it worked without any issue.

When I was loading the form on Internet Explorer I checked the console and I saw an error, that error was not there when I tried using the direct link to the form.

Could be possible to re-embed your form?
