When Submit Button is Hit, Nothing Happens

I have two different forms on a webpage - one form is fine - you hit submit, you get a thank you and I get the email with the form information.

However, on the second form, when you hit submit, nothing happens other than "please wait" . . . .

Help.

I see that there is an issue with the form on your website. When I tried to submit the form, I'm able to replicate the issue. Could you try embedding the form using normal javascript embed method on your webpage to see if that helps?
