Adding a Form Email Autoresponder

Hello, I have a question about the Submit Verification form - with email autoresponse...

 When I launch the app, and try to connect with one of my forms, all I get is

"This form does not have an allowed question. Please select another one."

Any clues on what might be needed?
or if this app is no longer working,

any other ideas on how to send AUTORESPONSE Emails to people who submit the form to me?
