I just checked your form and found out that your notifier is modified. Our tool that would auto update the fields in the notifier won't work when the template is modified. In this case, you have to manually update the fields to reflect the changes.
You can also re-create the whole settings. You have to;
1. Delete the current notifier (http://www.jotform.com/help/132-How-to-Delete-Notifications-Autoresponder).
2. Recreate it after (http://www.jotform.com/help/25-Setting-Up-Email-Notifications).
