Email to notify me of submission

    One of the fields is not showing in the body of the email that is sent to me when a submission is received. I have tried both a check box and radio button field but neither of the work.  Everything else shows.

    Please go ahead to include the missing field from the notification set up page, as follows:

    I have cloned your form and tested, got the following emailed:

    I noticed you are requesting credit card information on this form, please be advised that this is illegal as violates our Terms of Use. If you want to collect payment through your form, pleas include a Payment Integration, the following guides will be helpful:
