My forms have disappeared?

Hi, 

When I login and go to My Forms they are all listed (see Screenshot).  But when I try to edit one it shows that all my forms are blank??????  I have tried in both Chrome and Firefox

It appears that your account has not been used for a long time so it went into dormant mode. I have reactivated your account so you should now be able to see your forms. Please check on your end to confirm.
