My emails from this form are not showing up:
My assistant Shari is getting her copy, but I am not receiving mine anymore. The last one that I received was on 2/9/2016. For some reason, the email stopped.

It appears that your email address was added to the bounce list, which is why you were not receiving any emails. I have removed the email from the list so you should now receive the email notifications.
You can also check on your account if an email address is in the bounce list and remove it. Simply follow this guide: How to remove your email address from bounce list
