You have issues with your https security certification. It says that it was not renewed as of September 2015.
I had problems both using Google Chrome and Safari.
I had to accept login without https.
But funny enough, being on this page jotform.com/contact/ it shows https without any problem.
Please find screenshots of the secure alert from Google Chrome.

I was not able to replicate mentioned issue in Google Chrome.
Maybe it was temporarily issue. Did you try using some other browser?
If the issue persist please let us know.
