    How to populate Fields on the Form using link in the Email

    Can your system allow me to create a Form that will allow a user to click on a link to the form in an email and then the form will extract data from the link that was clicked and populate fields on the form and then allow the user to answer a few questions and then hit submit and the form will be emailed to a email address?

    Yes this is possible in JotForm. You can use the field prepopulate method using URL parameter value as described on this user guide:
