What is the issue you have encountered with your form? I just did a test submission and it worked:
If you are referring to the email notification not being delivered, it might be due to it's current configuration, the Sender E-mail is missing:
Please assign to it email@example.com. Also, check your spam folder, let us know if you need more help.

from the contact us page.
I checked the script on the page and the server
I changer the sender email to firstname.lastname@example.org
but I didn't get about checking my spam folder.

You are welcome! In addition to this, you may white-list our domain names, if the issue continues, as we use Amazon SES method for "email@example.com" sender.
