The reason why you are getting 2 email notifications is because you set up 2 email notifiers.
1st notification has 2 email recipients (firstname.lastname@example.org, email@example.com)
2nd notification has only 1 recipient (firstname.lastname@example.org)
So every time the form is submitted, 1st notification will be sent to 2 emails. After that, the 2nd notification will be sent on the same email address.
Please delete the either the email recipient on the 1st or the delete the 2nd notification. Here's a guide that you might need: How-to-find-and-change-the-email-address-used-for-notification-email
