Hi, we have patients fill out our forms and then hit submit which send them to us in the form of an email. The problem is they all come in as the name of the form, so for example we have 3 emails labeled New Patient registration, but we have to open each one to see who filled it out. Is there a way for them, when they filling out, to put a name somewhere so it will identify who is sending the jotform? Thank you!

I could not locate any embedded jotform in your site. I guess you are referring to this form: http://www.jotformpro.com/form/60094252636959
You need to go to the "Reply-to And Recipients Settings" of your notification, and assign a field of your form, where the user inputs his name, to the "Sender Name":
If you need anything else please open a new thread, we will be glad to assist you.
