Hello,
Not sure if the form is mobile responsive on other platforms, but it doesn't even show on IPhone 6. (Even though it is fine on IPad).
Help!

Hi,
I can see that you change url to your page when it detects you are coming from mobile device. I think that is linked with your problem. Whenever i get mobile version of your site form is not working.
Can you check if there is a problem with embeding your jotform in mobile version of the page?
You have multiple ways of embedding Jotform on your website. Could you try to embed your form via iFrame method or any other way of embeding.
You can embed your form via iFrame by clicking on Publish button on form builder.
Then click on Embed button and choose iFrame from embed options.
Copy code from gray text area and embed it on your site.

Hi,It worked. Right now we are using Godaddy's website builder....turned off mobile version and it works.

HI, THAT MAKE MY FORM GOING AGAIN IN ONE COLUMNS WHEN OPEN IN DESKTOP. PLEASE FIX YOUR PLATFORM TO WORK GOOD IN ALL WAYS. WE WASTE TIME GOING AROUND AND AROUND

Please embed the form using the default Embed code or the iFrame code.
However, to make it to work in mobile browsers, you must enable the Mobile responsive option, here is how: How to make Forms Mobile Responsive
