How do I retain the form question and form results after the form is filled out and a customer clicks submit?

Thank you!  For clarity, when I submit a test form (filled out) it doesn't show the questions or the complete answer.  So, what we need is for the questions AND the answers (from a completed form) to be e-mailed to us.  In this way, we can use the form for lead generation.  What do we need to do to accomplish this?
