The last two fields on my form are not making it on to the email with the responses. Can you help?

    Hello,

    I have take a look at the Email Notification configuration for the specified form and notice that the last to fields 'Coupon Code' and 'How'd You Hear About Us' are missing as you can see bellow. 

     

    The last two fields on my form are not making it on to the email with the responses Screenshot 20

     

    Please go ahead and add them and check your email to see if all the fields go through.
