You can see below my form and then the image shows what is sent when it is submitted. Not sure where the disconnect is taking place. Also, how do I get the form to appear as a notifier to look exactly how it is designed?Page URL:

You have more than one email notification:
Seems like in notification 2 there are fields that does not even exist in your form any more, such as "Do you own the home". So, I would recommend you to delete your notifications, and recreate them again, so all your form fields will be updated: http://www.jotform.com/help/25-Setting-Up-Email-Notifications
On regards of the design of the email notification, that's the default design, if you want to customize it too look like your form's style, then you will need do it through edit tools of the compose email:
