How can I rectify the website link of 'Referrer'

Please help to rectify website link in image attached.

This is 2nd time I have to add in 'Referrer' field again.

Base on the image you have provided, it seems that the URL that appears is not where the form is submitted, is that correct? Can you provide us the website URL where the form is really located/embedded? 

I'm not sure, but it seems that the script on your website and the script on your form are having a conflict. I would suggest you try to embed your form using iframe and make test submissions after.
