Can you please let us know your JotForm account username or the email address you have used in your account profile? The form number you have mentioned above seems to have been deleted already but we will check it once we are able to trace your JotForm account.

I checked everything to our end, your email, your IP Address, to our email logs, support email but nothing shows up.
Do you have any other form URL that we can use to check? Email Address?

I have received a submission from the form so it did exist.
Do I need to rebuild then?
I think the email from the form will help us to trace the account. Please get the full headers of the email and paste it here. In the email headers, you should be able to get the form ID.
