Não consigo mais acessar meu usuário, quando solicito nova senha informa que o username não existe.

It seems that you are trying to login using a different username. As I check the last edited form, I see that you are editing the form titled "queroFinanciar.com - Solicitação Empréstimo Pessoal". This form is associated with an account charlesu2004. Please try login using this username to edit your forms. If you have forgotten the password, you may try resetting the password from the login screen so that the password reset instructions shall be sent to the account registered email address.
