    Once saved, the form of the submission is jumbled.

    This problem just started recently. Once I have a submission, I open and save it to my desktop. Once I go to open the form from my desktop, questions 2-9 are jumbled. The questions are on the right side of the screen, the answers on the left side. It is hard to see what answers go with what questions.

    I checked the submission data but there is none at this time.

    The PDF Report can be reset in the Submission page using the Customize PDF option in Get PDF whein viewing existing submission data.

    Please provide more details and we will check further.

    I ran into this and fixed it by changing the size of the fields on the PDF. Two things I did:  First, extended each field so that the box for its data reached all the way to the right-hand side of the paper. (Sometimes date or quantity fields were short.) Second, there is a character on the lower-lefthand corner of each "box" - try making the first field that's displaced a bit deeper by clicking and dragging that character. You may want to enlarge other fields accordingly.

    Looking at your PDF above, I might suggest deepening the field containing "None/No Changes."

    Not an expert - just another user making a suggestion.
