    adding a logo to the PDF output of the form submissions

    I am having trouble inserting an image to the PDF stigmatization platform. I am able to insert the image but it instantly messes the order of the printed fields. The preview panel shows the PDF as it should be but when i download it it all messed up. please help me.

    Are you referring to this form (http://www.jotform.com/form/60523465998973)?

    cloned that form and tried to add an image using the customize pdf feature. 

    After downloading the pdf, I was able to replicate your issue.

    I was able to  change the appearance by reversing the positions of the fields and their answers in the PDF designer. Simply drag and drop the fields to the position where they appeared when you downloaded the pdf.

    This is how the downloaded pdf appeared afterwards.


    Simply do the same to your form and after some adjustments, the pdf should now be fixed. If you need further assistance, please let us know.
