We use a Google Docs spreadsheet to keep track of our JotForm submissions for on boarding purposes, and the sheet has two tabs. Recently, the second tab has been deleted each time a new form was submitted. I'm uploading a screenshot of the tabs. Please let me know what we can do to solve the issue.

I cloned your form and integrated that clone with my own google spreadsheet. After i created a new tab, I sent several submissions to see if the tab would get deleted. It appears that I cannot replicate your issue as the second tab did not get deleted during my tests.

I would suggest you reintegrate the form with google spreadsheet to see if that fixes the issue. Simply remove the previous integration, clear your browser cache, and reintegrate it again to google spreadsheet.
